: The risk for HIV is extremely low but Hep C infection is still possible because HIV is easily killed with the disinfectant(s) but not Hep C virus. However, in order to contract to Hep C virus you have to assume the equipment had been contaminated with Hep C virus before it was used for you so it is still a long shot.

Black spot: It may well be fungus again.  See your podiatrist for an evaluation and treatment.

Neosporin ointment : topically, boil your nail clipper and after it cools down, wipe well with rubbing alcohol. BP is normal.

Yes: You're in philadelphia.  There is a podiatry school and a bazillion podiatrists.
Go.  If needed, have the nail removed and take a course of lamasil (terbinafine).

Apply antibiotic: Cream and bandaid, if pain persists, warm water soaks.

If it's not too deep: Apply pressure to control bleeding. Apply topical antibiotic and bandiad or dressing. For more severe cuts, get medical attention and have it checked out.

It may be fungus: But doesn't have to be, that little toe is often subjugated to trauma from shoe and all the microtrauma can cause thickening and slight discoloration. 
The only way to know the true etiology is to have a sample of the nail sent for pathology. (A podiatrist or dermatologist frequently do this.)

is it bad to cut the dead skin around and inside your toe nail cuticles?

No: There is no harm in this, unless you are a diabetic.  Just be careful not to cut yourself and to soften the skin prior to cutting the cuticles.

Ingrown toenail: when toenails are trimmed too short they can impale the flesh as they grow forward causing pain, redness, drainage. Trimming the offending border out can often cause it to become worse if you don't get the entire margin out. It can also cause it to have temporary relief only to return , again, when the nail grows. Seek medical attention from your local podiatrist.
