when they biopsy terminal illeum in regards to a crohn's diagnosis. what are pathologisy looking for to confirm diagnosis? and i'd you can't get diagnosed from pathology.. what else could it be?

i had lowar lumbar  pnuemonia for a month then ate a undercooked burger abdominal pain and apendicitis then crohn's diagnosis was i misdiagnosed.

Not likely: Your case sounds complicated. Appendicitis can occur in any clinical situation and is treatable, curable and occasionally has serious complications. Crohn's is not curable, frequently has serious complications, and your health care team will be very happy if you don't have it. As doctors we search for the diagnosis that explains the clinical situation, we are not always correct. Be well.

Need more info: From what you described, you have inflammatory bowel disease. It is not feasible to narrow the diagnosis further, without additional information, especially examining the tissue that was removed. It may take a collaboration between, pathology, radiology and gastroenterology to sort it out.

They can be used to assess the severity of any inflammation. Colonoscopy is the most important.

Not a bloodwork call: Routine labs reflect only the illness (malabsorption, inflammation) and are nonspecific. A pricey antibody panel is offered to perhaps support and/or prognosticate the diagnosis. The diagnosis is made on imaging and perhaps biopsy or (worst case) resected surgical specimen.

Biologicals is bette: High dose prednisone is not a safer option though very cheap. Biologicals if introduced early will help to prevent complications of Crohns. It’s very costly still. Almost 5000$ a month. Need to do TB tests once a year.

The first step: is a complete history, physical examination and basic lab tests (Blood Count, Chemistries) There is no BLOOD SCREENING test for these disease. A stool examination may be helpful( testing for occult blood)  Routine genetic testing (HLA chromosomes etc) is NOT recomended. The next step for most Gastroenterologists is ENDOSCOPY (Lower and Upper) which can be done in ONE session (painless  nowadays)

i am having stomach issues for the past 5 weeks. i may have crohns disease based on scope, but diagnosis is hard as this long for doctors. i did a crp test at a clinic and had an 8. is this high inflammation , specifically related to crohns?

: Crp is a nonspecific test. Not diagnostic of inflammatory bowel disease. Speak with GI about your concerns. If your current GInis not helpful, seek a second opinion.
