See ORS: Not sure where your old fx. Is located( usually thoracolumar)but not entirely uncommon to be picked up on a radiograph years later.Possibly had an accident in the past and forgot about it or attributed it to 'strain' or other. At any rate, see an ors now who specializes in spinal disorders.Flex/ext. Xrays, MRI and quality physical exam needed. Possible old fx.Not cause of pain now. Gl!

Conservatively: If it is causing pain, with gentle pain medications (nsaids like advil or Aleve unless contraindicated; or if more severe, tylenol (acetaminophen) #3). If it is not causing pain, most would leave it be. No surgery is indicated, especially in a 93 year old.

Yes and no.: It is unusual for an 18-year-old to have a compression spinal fracture. Im an 18-year-old one would expect either a traumatic reason or an underlying pathological condition. A compression fracture without incident, usually seen in a more elderly population with osteoporosis for example as a cause. See your orthopedic spinal specialist for an evaluation.

Conservative: The majority of them are treated conservatively and dont require a brace. 
It depends on the fracture pattern

very rarely is cement augmentation (kyphoplasty, vertebroplasty etc) is needed for patients with unrelenting pain.

While the pain can: Decrease, and the bones can heal, the compression of the vertebra does not normally occur. The wedge deformity that occurs from a compression fracture will usually remain until it is fixed with a vertebroplasty or surgically. Bracing can help a great deal.

How did it happen?: If it is due to trauma, once it has completely healed, the likely future issues would be with degenerative joint disease, or neck "arthritis", which would probably occur sooner because of the trauma.

If due to another cause, more details of that cause are needed.  Please speak with your doctor or a spine specialist for more detail.

No: Not necessarily.  Some people with vertebral fractures don't have pain.  The L1 fracture may be old therefore not causing you pain.

Vertebral : Vertebral compresion fractures are very common in post menopausal women especially caucasians. The fracture may or may have not been related to the accident. Two ways to see if the fracture is new or old are to either do a bone scan or an mri. If the bone scan is hot the fracture is new. The MRI may show blood around the fracture site and associated swelling if it is a fresh fracture.

Vertebroplasty: There is a minimally invasive procedure that is out patient for compression fractures.  Most patients are candidates if they have pain and no spinal cord compression. See a neurosurgeon about kyphoplasty or vertebroplasty. Cement is injected into the vertebra to prevent further collapse and help with the pain.
