PBC = inflammation: Primary biliary cirrhosis is an autoimmune inflammatory disease, manifest by low but persistent liver damage.  This causes a variety of biochemical changes which manifest by fatigue.  Not all patients have fatigue, many patients are asymptomatic until the disease is advanced.  Cirrhosis is also associated with fatigue.

Treat the cirrhosis: Some forms of cirrhosis have specific treatment, like viral hepatitis and primary biliary cirrhosis, and all can be treated with transplanation. Avoiding alcohol is critical, even if it is not the cause.  Beta blockers like Inderal (propranolol) or Nadolol can prevent bleeding. A healthy diet with reduced salt and modest protein (40 grams/day or less) is good, and you could try milk thistle. Itdoesnt cause pain.

Many things: You already abstain from alcohol! sleep 8 hours. Eat enough protein: 1/2 gram per pound of body weight daily. Alpha lipoic acid, phosphatidyl choline, coenzyme q10, b vitamins, milk thistle and n-acetyl cysteine are all valuable liver supports.
You may want to have your doctor evaluate your ammonia level. There are medications to help if that is elevated as often occurs in cirrhosis.

Sorry to hear: of your husband's trouble...the MOST IMPORTANT "start" dealing with this situation is the complete PERMANENT CESSATION of ALCOHOL in any form! Much of the liver damage may be REVERSIBLE .. Your husband should seek help with DETOX...(If he is already SOBER...vitamins and good nutrition and time may lead to reversing the symptoms/signs . If he continues to drink he will die!
