Healing scars: A scar forms when there is  healing of an injury to the tissues.  Remember that with almost all wounds, healing continues for months and months as we change the stress on the tissues.  A scar forms as a result and may have many different appearances and colors.  Any dark coloration may fade over time but the tissues never are the same and so some scarring will always be there.

Time: The scar will fade with time. Let it take its natural course.
For good health - Have a diet rich in fresh vegetables, fruits, whole grains, milk and milk products, nuts, beans, legumes, lentils and small amounts of lean meats. Avoid saturated fats. Drink enough water daily, so that your urine is mostly colorless. Exercise at least 150 minutes/week and increase the intensity of exercise gradually. Do not use tobacco, alcohol, weed or street drugs in any form. 
Practice safe sex, if you have sex.

Scar is always...: A scar is always the residual sign of skin/tissue healing after cut/injury. So, if not related with circumcision, id you have a pimple-like "boil" before? If concerned, go and see doc timely. But, if not hurting and with no erosion or size increase, it is benign with no need to intervene but personal finicky cosmetic reason.

Yes, if you look: Yes, scars are permanent. Usually though the scars will blend in and are not noticeable unless you look for them. Also depends on location and how the injury occurred or by whom the wounds were treated that resulted in the scars.

Probably not: Could a circumcision scar be hypertrophic? I have never seen or heard of this happening but in medicine we frequently say that anything is possible. I would add that it is highly improbable though and I have asked collegues and none of them have ever heard of or seen it either. I probably doesn't happen.

Methods that are likely to work do so by increasing the normal skin turnover. Laser scar treatment is the easiest and fastest and quite safe.

Circumcision: A revision can be done - see a urologist for an evaluation.  Do not put any material around the penis to retract the excess foreskin as it may cause loss of blood flow and potential gangrene or necrosis.  A patient of mine was bothered by his foreskin and wrapped a rubber band around his penis which nearly cut off his circulation - never do that!

Comedones: What you describe are likely common skin lesions known as comedones.  While tempting, "popping" them can lead to infection under the skin in the area of the hair follicle where the sebaceous glands  ( where the cheese-like sebum is produced) are located. When exposed to air the tip of the sebum core turns black.  Hence, the nickname "blackheads".

Nothing or...: In general, there is no practical need to remove any circumcision-related scar. But, before doing so, be evaluated with detailed history + physical exam first so to decide the balance of its medical necessity and professional possibility. If really needed, it can be done under local anesthesia as an outpatient.
