Get treated!: The natural course of HIV is variable and universally devastating. Yes, some infected individuals will nosedive rapidly and others go years without symptoms. Predicting which opportunistic infection will manifest 1st and waiting for it is foolish. Don't even consider CD4 levels as a guide to treating this virus. Antivirals for HIV are extremely effective. Every HIV infection should be treated now!

CD4 lymphocytes: In order to know if it is possible to bring that CD4 number up, we need to know the etiology of your low CD4 or CD4 lymphopenia. Most common causes of having a low CD4 count is HIV infection where the virus infects CD4 lymphocyte preferentially and destroys it faster than it being made in the bone marrow. Malignancy, genetic mutation, infections can cause it too. You need to see a doctor.

HIV: This means your arv is not working well. Either you are not compliant or the virus has mutated and become resistant. This needs to be discussed with your doc. Getting a therapy that is able to suppress the virus is the only way to raise your CD4 count. Will it ever go back to what it was even on perfect therapy - that's debatable, but possible.

Yes: A change from 75 to 72 is no change at all - the test is not that accurate.  If the test was run on the same sample twice in a row it could indicate a similar variation.

Viral infection (including coronavirus) may affect the lymphocytes in your body, so these changes can be due to coronavirus.

The CD4 count is very low and you are prone to “unusual” infections.  See an HIV doctor
