: Large variability, specifically if more than one mechanism involved in symptoms, day or 2 to no response/ partial in case infection is resistant or symptoms are due to other reasons.

Comprehensive ...: Better to see PCP and have a comprehensive History, Physical, labs and appropriate Imaging studies to have exact diagnosis of what seems to be a recurrence of Symptoms. Then see the specific Specialists like Gastroenterologist or Infectious Disease  Consult as the case may be.

i had esophageal candidiasis, found in endoscopy. it was treated with 3 weeks on diflucan (fluconazole). how do i know if it is gone?

Esoph candidiasis: Generally this doesn't happen with a normal immune system and is usually found on endoscopy -  have the symptoms that you were having resolved?    Generally 2-3 weeks of Diflucan (fluconazole) will take care of the problem.    The alternative would be to repeat the endoscopy.

endoscopy showed normal esophagus but esophageal brushing/smear culture showed non-albicans candida growth? is it possible to have esophageal candidiasis without white lesions or inflammation seen in biopsy?

: Candida whether albicans or non albicans are part of the gut flora so it is possible to culture it because it is a colonizer rather than a pathogen. It is when there is overgrowth and because of change in flora pattern that it could cause disease and the white patches.

for esophageal candidiasis that improved with low dose (100 mg) during a week of fluconazole but came back and persisted despite restarting treatment at higher dose 200 then 400 mg, would 800 mg be safer or 200 mg itraconazole / voriconazole?

: Oral thrush is usually a liquid suspension of nystatin, Intestinal yeast infections can be treated with the pill form.

Call a doctor : Nystatin is available by prescription only. Need to speak with a doctor to get. 
Candida resistance to fluconazole is extremely rare and very unlikely. May be another condition entirely.

after an acid reflux about 6 month a go. i was diagnose with candida infection in esophageal and i still feel dull chest pain about few times a day.?

Get checked out: Candida in your esophagus is not common without some underlying problem. I assume you were treated for the candida, but you'd also want to know why you got it. You should see a primary care and/or infectious disease doctor about it, especially since you're still having some symptoms.

do candidiasis also cause severe back pain?

Not usually: Yeast infections usually do not cause back pain - except in the case of people who get a fungal infection from a contaminated back injection.  Look up in the recent news about steroid injections contaminated with mold.
