Definitely not: Lack of nutrients is also not very likely since the enamel is fully formed prior to the tooth erupting into your mouth. More likely there is undiagnosed clenching or grinding or perhaps  previous trauma that may be so long ago that it seems unrelated.

Absolutely!: The trauma that cause the chip in your tooth could have damaged the nerve and blood vessels inside the tooth. As the tissue in the tooth dies, it will literally stain your tooth from the inside out giving it a dark brown or blackish color. The tooth should probably be treated with endodontic therapy as soon as possible before  the infection spreads. Please go to your dentist.

See Dentist: Recurring lesions are not normal.  It would take more than 400 characters to list all the possibilities, most benign, but some not so.  Don't be foolish.  Take what you call "ulcers" into your Dentist or an Oral Surgeon and take advantage of their years of training and experience.  Self-diagnosis is at best dangerous.

Chances : Chances are the chipped baby tooth will not affect the permanent tooth that will come in later. Her dentist will evaluate what treatment is needed, and monitor the tooth at her check up visits. If the nerve in the baby tooth goes bad due to the trauma, the tooth might need a "baby tooth root canal". This will save the tooth and prevent an infection in that area, which could be problematic.

But right now, don't worry too much about it, because there is not much you can do. As long as she is comfortable and not in pain, i'm sure she'll be just fine.
