No infection: suspected. Need infection to get Osteomyelitis (spread of infection to bone) sepsis (infection that has spread to the blood stream) or a brain abscess.

Yes: If it is in the cervical (neck) region.

Great question: In patients who have not had surgery and now possess osteomyelitis, the typical cause is the spread of bacteria through the bloodstream. Even brushing our teeth can push bacteria into our bloodstream. Sometimes those bacteria land on a particular tissue and start an infectious process.

Yes: An abscess does not necessarily/usually involve bone.
Bone infection does not necessarily cause an abscess.
Abscess is a walled off pocket of dead tissue (pus) somewhere in the body.

Contiguous infection: Chronic osteo can cause the surrounding soft tissues to get infected and may lead to what is called a sinus tract, which is a tunnel that leads to the skin. I am not aware of other issues.

Possibly: If there is a long term chronic infection over the bone, especially in the setting of immunosuppression, diabetes, or poor blood supply, it is conceivable that the infection could spread to the bone.  Of greatest concern would be skin flora such as staph (including mrsa).

Not realy: It's estimated that between 10 and 25 percent of people with monoclonal gammopathy of undetermined significance develop a more serious condition, such as multiple myeloma or other cancers or blood disorders. Other complications  include fractures and blood clots.

Jaw osteomyelitis : multiple causes. Some of the more common would be untreated dental or sinus infections. Also injury, injections or prior surgery to repair jaw may cause osteomyelitis

Yes: That is possible   it would be called a "drug fever"

Possible: Something as simple as brushing your teeth can introduce bacteria into your blood.  Also, an infection anywhere else, can sometimes get in the blood and travel.  The bones are very rich in blood supply.  You are right that most times there is a definable reason, but spontaneous infections do occur.
