Most commonly it: Is from a foreceful motion with a sudden change in direction which applies a torque to the tendon.  Examples are a sudden change in direction when playing a sport such as tennis or basketball.

No: Intermittant steroid injections for a ruptured disc or epidural injection will not predispose you to tendon rupture in the spine or cause ill effect unless you are having them done repeatedly and at less than 3 month interval.  Worst case scenario is that it doesn't work.

Yes and no: Not in anywhere near the same mechanism as with tendons, but all antibiotics can potentially cause some degree of stomach and GI problems by killing the normal bacteria that populate the GI tract.  This can cause thrush in the mouth and diarrhea in the GI tract by over growth of pathological bacteria.  Don't hesitate to be seen if you are having problems.

Risks and benefits: All medications have risks and benefits. Bactrim is a medication like Levaquin. In general, medications are in our body (eg. Liver or kidneys) which may limit the amount of medication. I found retail drug store pharmacists are a good source of medication precautions and may be able to provide a print out for your review. Bactrim is a drug that has been in the market for a long time.

Direct trauma: Tendon injuries are very catastrophic.  Usually occurs when the foot is in a fixed plantargrade position and the tibia (shinbone) externally rotates rapidly, and the foot does not release from its flexed state.
Injuries for tendon ruptures also take the longest to resolve, which can be from 8-20 weeks, depending on the amount and time of damage to the tendon.
