i had cataract surgery 2 years ago and a toric lens placed in my left eye. it was never right. can it be redone? i had lasik surgery in the same eye.

Probably not: Repositioning a toric iol  is easily done in the early postoperative period. However 2 years later entails significant risk due to adhesions that have formed between the iol and the lens capsule. An examination to determine if the axis of your corneal astigmatism aligns with the astigmatic axis of the iol  needs to be performed first to diagnose the reason for your poor result before any treatment.

i had cataract surgery in both eyes a year ago. i am left with a terrible astigmatism in my left eye. the right eye is fine. can the toric lens he used be removed and a standard lens be put in?

Accepted Answer

Yes: The options can include a toric hard contact lens vs. Glasses vs. Removal of the toric lens and placement of another intraocular lens.  A second surgery is much more difficult and has a higher complication rate.  Depending on the situation, you may need to get a vitrectomy surgery in addition and you my need a rigid contact lens regardless if there is significant corneal abnormalities.

Good option: For those with moderate to significant astigmatism, which is a good portion of the population, choosing a toric lens implant for your cataract surgery will reduce your dependence on glasses post surgery. I advise my patients who have astigmatism to opt for a toric lens if they can afford the premium cost. If they can't, glasses will correct the astigmatism post surgery.

Type of Lens Implant: Your best lens is the one your surgeon is most comfortable with. Patien do have choices if willing to pay extra for the lens implants that correct astigmatism and theone that give you distance an near vision.
Other considerations are if you are nearsighted and prone to retinal detachments, silicone materials are avoided.
Intra-operative complications may dictate which lens is best then and there.

Yes: With the right selection of patient to receive this lens and the proper expectations of the type of vision to be expected, this has been a helpful addition to this surgery.  It is gradually becoming more commonplace and the available lenses are continuously being improved. Note that it is an extra personal expense not covered by insurance or medicare.  Discuss with you eye surgeon.

No change: Once you are healed up after surgery, your eye should feel normal. With the accommodative lenses, there can be some minor feeling of eye strain as the muscles strengthen to improve your reading vision.

Likely lens exchange: Choosing the correct refractive power in the lens implanted in the eye during cataract surgery is vitally important to restoring clear vision. If the incorrect lens is mistakenly implanted, typically a second surgery to replace the lens is required. On occasion the incorrect lens is similar enough to the correct lens that minor glasses or even laser surgery can be done to correct the error.

Wrong lens: Yes, but it depends how wrong it is...  It is a difficult surgery, and more things can go wrong.  If you just need a little glasses to correct the differenece that might be all you need.  Alternatively, if your surgeon has little experience with this, you may want to seek out another surgeon that has more experience removing or exchanging lenses.
