Often : For individuals with hypertension alone, taking no more than 1 to 2 blood pressure lower medications and with stable blood pressure and who are asymptomatic, kidney function can be checked once to twice a year. More frequent measurements may be indicated depending on your risk factors.

Odd: Water pills ought to be taken in the morning, not night, so that you can sleep at night.  Taking a water pill at night will disturb your sleep.  Further, I am not sure what your additional risk factors for heart disease are, but Toprol (metoprolol) XL would not be my go to 2nd drug, rather an ACE inhibitor.  THE ace inhibitor and HCTZ can be taken together in the morning.

A murmur is just: a sound the blood makes as it passes through the heart. There are many different types of murmurs and most of them are completely benign (not indicating disease.) Things can change and people can develop a  murmur because of disease but it is unlikely at your age. Hypertension can cause a murmur as well. Have another health care professional confirm the findings before you become too anxious.

History & physical: If you don't think you're anxious / upset, look further. Do get treated for the vitamin d lack. A good primary care physician can tell whether this could be hyperthyroidism, pheochromocytoma, aortic regurgitation, medication effect, primary cardiac disease... And direct referrals accordingly. If you're not satisfied, get with a good cardiologist. Best wishes.

Pain feels bad: Pain will raise a person's blood pressure. Pain hurts; pain feels bad. That makes one's heart rate increase and blood pressure rise. One's primary care doctor can look at the whole picture and offer help. Specialist care may add treatment ideas too.

Change diuretic ?: You may need to change the diuretic from spironolactone to a mild non potassium sparing diuretic temporarily to bring the potassium level down and at the same time not to get too much dehydrated. Discuss it with your primary care physician and see what will be the response.

Not "cause": In reality we do not KNOW the true cause of high blood pressure. We do know many things that will drive it up in a predisposed individual. Considering about 30% of population has HTN, it is fairly common. Smoking and/or any act that stresses the body or mind can drive BP up in someone wo already has HTN or is genetically "ripe" to develop  HTN

NO: You are fine. Relax. This is normal BP and Pulse. There is normal range of BP from 90/60 to 140/85. and Pulse varies from 60 to 100.
