Usually nothing: If pain/discomfort resolves after manual reduction.  In rare cases, you can get "en masse" reduction, meaning the sac gets reduced into the abdomen without release of the actual obstructive point.  But the pain usually persists of this is the case.  If your hernia requires frequent manual reduction, time to get it fixed.

Don't: C a surgeon first or ur pcp. 2 much pushing may cause bruising and swelling of the hernia contents & then it may become a surgical emergency.

Yes: Spigelian hernia is one at the semilunar line.  Reducible, incarcerated, strangulated are the 3 types of hernias.  You seem to be describing the first one... Be careful.

Recheck: Have the surgical site checked by your surgeon for Seroma or recurrence of Hernia.

Pre op imaging: During the process of evaluating this new lump, possible hernia, ask your team for a ct scan of your abdomen/pelvis. This imaging study will help your surgeon plan a hernia operation. The weight changes might not be related. If you have fever or vomiting get evaluated sooner. Good luck.

Doesn't always hurt: To answer your question, a hernia doesn't always hurt, and some people are able to push the hernia in, or "reduce it"  without discomfort.  The problem is that over time the muscles holding the hernia "in" can become stretched causing the hernia to become larger, or worse, the hernia can become "strangulated" where it twists on itself, requiring emergency surgery.  It is best to repair these early

: Once an inguinal hernia begins hurting,it's time to see the surgeon for evaluation and advice. It's urgent if the hernia can't be pushed back in ( reduced). Complications of incarceration and strangulation are dire and must be avoided.

Sometimes.: Umbilical hernias are small holes in the abdominal wall at the belly button through which the inner layer of the abdominal wall, the peritoneum, protrudes through to create a sac.  This sac has many pain fibers and is often sensitive to the touch, especially if it is difficult to reduce.  Your baby, regardless of their age, will let you know if it hurts, typically by grimacing and crying.
