Maybe: Osteoarthritis is the wears and tears of the body.  Anything that will put extra stress on the joints/bone/body can cause osteoarthritis, for example, weight gain with diabetes, thyroid or endocrine problems.

See below: There are a number of neck problems like yours, including dystonia, spasmodic torticollis, benign fasciculations, etc.  Not usually directly related to diabetes.  If it does not depart over next 10 days, gets more flagrant, or difficult to control, have your doctor take a look.

It : It is very difficult to say what type of rash this is without actually seeing it myself. Also I am not sure what you are referring to when you state "diabetic rash". A common cause of rash in diabetics are fungal infections. In addition a dark rash usually found on the neck and under the arms in diabetics is called acanthosis nigricans, this is due to Insulin resistance. If the rash is itchy then consider trying over the counter Clotrimazole cream twice a day. Otherwise follow up with your doctor.

Hard : Hard to tell without a clinical examination in regards to the neck (there are many possibilities such as acanthosis nigricans) so i would see a dermatologist.  Read about skin tag treatment at the link below.  I hope that helps a little.

I assume you had DKA.: A sudden onset of neck and shoulder pain would be much more likely to be related to a cervical disc issue. Unfortunately these symptoms sometimes are related to cardiac issues as well. It sounds like you need further evaluation
