Orthostasis: A sudden darkening of vision, known as a transient obscuration of vision (tvo) is most likely a symptom of orthostatic hypotension, or postural low blood pressure. Since you have some hearing loss as well, the first area to investigate is the vertebrobasilar arterial system of the brain, so a visit to the neurologist is recommended.

could a blood clot in the sinuses of brain/neck causes worsening two years of on & off neck, inner ear pain and excruciating headaches?i have iih.

Unlikely: A blood clot would get resolved in a few days or a few weeks at the longest. 
It is not feasible to provide a meaningful opinion without additional history, physical examination and may be some tests. It would be prudent to see your doctor.

Rarely: Sinus infection and middle ear infection may co-exist with the cause being an upper respiratory tract viral infection.  However, it is not common for sinusitis and inner ear disease to co-exist.  Inner ear infection should lead to vertigo and/or inner ear hearing loss.  Do you have the latter symptoms?

Two different bones: The inner ear is located in the mastoid bone, which is behind your ear on each side.  The maxillary sinuses are in the maxilla, which is the cheekbone on each side.  While they are only six inches apart, they are not really close

True: given also that you are dizzy and lightheaded, not a good idea to use an antihistamine, but you need to check your blood pressure. Saline nasal washes would help for now, probably nasal steroids as well, but ultimately you need to see your doctor / ENT specialist, good luck

i get a pulsating feeling in my head, sinuses and my top gums. the pulsating is in sync with my heartbeat. i know have bilat anterior canal bppv and inner ear fistula's .. what could it be causing it?

?: Your symptoms are varied to the point that it would be wise to see your family physician. Most likely sinusitis could be the reason but you need an exam to be sure.

6 weeks post septoplasty/sinus widening/polyps removed. normal to feel inner ear popping/tickly sinuses, swelling after  zpack/prednisone? worried!

That is normal: You are experiencing some ear eustachian tube congestion causing the ear popping . The tickly sensation in the sinuses is probably normal healing with some dryness. Make sure you are irrigating regularly with a neti pot or some other sinus irrigation system. If you are extremely concerned, see your surgeon again or call for advice.

Don't overtake Ned's: While calcium is frequently beneficial, it like all meds, need to be taken according to necessity and proper dosing. Too much of anything is bad.

Dizziness: Dizziness can mean 1 of 2 things; lightheadedness &amp; vertigo. Lightheadedness is feeling like you're about to faint or pass out &amp; may get nauseated. It is relieved by lying down. It is not serious &amp; usually is caused by drop of blood pressure or blood flow to head when u get up too fast. Vertigo is feeling like you or surroundings are moving, as if u r spinning or falling. It is most likely anxiety
