Blood test: There is no simple blood test to determine if a patient is taking any medicine for blood pressure.  Usually I can tell if a patient is taking their medicine because they are requesting refills at the appropriate time.  For example if you are given thirty pills, but don't need a refill for 90 days, well then you likely have missed some doses.

Maybe: Its up to your doctor. If you have a history of hypertension, then chances are good you will have your pressure taken. If you pressure is normal then he may just discuss the results with you.

ONCE IN THE BEGINNIN: You do not need periodic blood tests if you are being treated for high blood pressure, unless some abnormality was found on the initial exam
blood test should be done at the initial visit and needs to be repeated only if you are being treated with diuretics and we need to check your potassium level, otherwise once a year may be all you need.

Blood test will not.: I agree that you should have BP taken lying down, sitting, and standing. A blood test is for other things, such as serum sodium. It all depends on the symptoms.

Very Few: Medical conditions are associated with such a wide disparity in BP measurement within a 5 minute period. Unless associated with severe headaches and other neurologic and systemic symptoms, taking accurate repeat measurements that show the same differences will be necessary to sort out the causes. In that case, immediate consultation with a pcp is warranted.

Kidney disease: From a blood test, I suspect your internist is checking your BUN/Creatinine, makers of kidney function. Some BP medicines affect this parameter adversely and hence one needs a baseline value.  Alternately, your doctor is further looking at GFR and/or microalbinuria to ascertain microscopic kidney disease. Either way, BP control is important. Yes: A well conditioned athlete may have heart rate in 50s. Systolic blood pressure in 80s is fine if you do not have symptoms. What is your BMI? If it is less than 18 you may wish to eat more. Your dizziness may be due to dehydration, if you are not drinking enough water and if you lost weight recently that may contribute to dizziness.
