What have you tried?: If the "big guns" including systemic antibiotics, spironolacton, and Isotretinoin failed, you probably do not have acne but one of its mimics. A visit to a different dermatologist might be a better choice if you do not have other endocrine symptoms.

Frustrating issue: Although some consider acne a teenage issue some by their genetic nature will deal with it into their 30's and beyond whatever their gender. Hormonal manipulation for treatment is a rather recent addition to treatment. The suggestion your may change after pregnancy is a guess.Find a dermatologist interested in adult acne &amp; develop a personal treatment plan. I doubt any Endo would be interested.

Primary care: This suggests polycystic ovarian syndrome which is characterized by irregular periods, excess hair growth and acne.  Basic lab tests and an ultrasound study are very useful in making this diagnosis, however if you're unable to afford these, a trial of birth control pills may be helpful.  A local family physician or planned parenthood clinic may be able to provide this for you affordably.

If : If you have normal periods i would not be concerned about hormonal imbalance. However if your periods are irregular then you could be dealing with this issue. At this point, i don't think you need an endocrinologist. If birth control managed your symptoms previously then this is worth another try. There are usually many low cost or free clinics that provide birth control. Lastly, get a yearly physical with a primary care provider when you are able to do so and that provider will guide you through this situation.

It is likely that some blood test and other exams will be required by your endocrinologist. It is possible that interdisciplinary work between specialties will be required, please be confident.

Not proven: Goldenseal is a herb found from the northeast to Arkansas. It contains  berberine which has been used in several medical conditions without proof.  Most of the constituents  in Goldenseal are poorly absorbed and amounts off berberine are variable. There are much better treatments for acne available so check with a dermatologist
