Tight fitting shoes : May impede circulation or put pressure on certain nerves in the foot which can add to the effect of neuropathy or cause numbness by itself .  Nerves need blood supply to work well and that includes blood flowing in and out of the foot ( or anywhere in the body) tight pressure on sensitive nerves in the ankle  or between the toes can also cause nerve compression or neuroma formation giving numbness.

what is the operation called to correct a large bladder that due to age and maybe nerve damage, no longer voids enough. do not want to use a catheter for the next 20 years. folks live a lot longer now and i have great blood work test results?

: I wonder if you talk about a procedure for urinary diversion which is done to create an ileal conduit by using a section of ileum to create a pouch which serves as a bladder and two ureters are connected to that pouch with opening on the lower abdomen. The urine will be collected in a bag, almost the same as a colostomy. This procedure is usually accomplished by a general surgeon and a urologist.

Accepted Answer

There : There are a few nerves around the pelvis that could be injured during graft harvest.  The most common one is the lateral femoral cutaneous nerve which supplies the skin on the outside of the thigh.  From your description, it is more likely the ilioinguinal or pedundal nerve that is affected.  Since i don't know where the bone was harvested (anterior or posterior crest), i don't know how the nerve might have been injured.

Most of these nerve injuries are what are called neurapraxias.  The nerves recover with time, but can be painful for several months to a year.  A charring injury from the cautery used during surgery also recovers with time.

If the nerve is partially (axonotmesis) or completely (neurotmesis) severed, the recovery is less predictable.  The nerve fibers have to regrow and find their way to the nerve endings.  If they "get lost" they can cause a painful ball of nerve fibers called a neuroma.  Nerves take a month to start growing and grow at 1mm/day or about an inch a month.  Based on the location of the injury and the distance to the nerve endings, it can take a long time to regenerate.

Injured nerves can be very irritable and send any stimulus as a painful signal.  To quiet the nerve, we use desensitization techniques.  By constantly giving the nerve a low level stimulus, it "learns" what is normal and stops the painful signal.  Then you increase the stimulation until that feels normal.

While Neurontin can be helpful, I have had better pain control (though not necessarily side effects) with lyrica (pregabalin).  Insurers are often reluctant to cover it because it's more expensive.  Amitriptyline or nortriptyline can enhance the effects, but also add to the sedation.

It is possible you may need surgery to explore the nerve and remove a possible neuroma, though that would likely leave an area of numbness.  Only your surgeon knows what the likelihood of that is.  Nerve conduction studies usually are not very helpful in this area and can be painful to get, particularly if the nerve is quite irritable.

To date we do not have any means of making nerves function.  We can decompress them, repair them, remove neuromas, and even transfer them, but they have to work on their own.  Maybe in the future we will have nerve growth factors or stem cells to help boost nerve function.  For now, however, nerve pain remains a frustratingly difficult problem.

how about fermoral neuropathy. i'm a type 2 diabetic with servere nerve damage to the left ankle which was caused by a slip and fall to my lower bk ?

Diabetes or Trauma ?: There are several types of neuropathy, including femoral or amyotrophy, which can affect patients who have diabetes. It is possible that you may have been or presently are having symptoms associated with them. The bigger issue is separating the damage caused by the physical trauma the ankle as well as the lower back. A neurologist might be the one to best sort out the various possibilities.

broke my foot in september 2022, fully healed now, but i wore a brace, and it caused trauma to big toe, and now constant numbness and toenail, possibly affected, and not sure if will fall off. will i regain feeling or have permanent nerve damage?

: Pressure on your big toe with the brace can cause it to be numb for quite awhile, even a few months. The toe nail falling off is not uncommon if there was butting up of the toe at the end of the toe. This can even happen with running shoes that are too small and constant butting up of the end of the toenail. The nail usually grows back and is not a prooblem.

gp diagnosed alcohol neuropathy 3 wks ago by call.had withdrawal. worried anxiety is mimicking the tingling in feet now.on sunday a&e doc did strength,touch,reflex tests,heart trace.sugar level.said pyhsically fit. anxiety the cause.suffer badly?

Maybe not: With all those in-person tests being normal, risk of peripheral neuropathy is pretty low.  Can’t tell for sure without a more careful exam by a Neurologist.  Check with one if you are still worried.

Just perhaps, guidance regarding long-term withdrawal from alcohol may be more beneficial

Not so: TMJ may cause nerve damage but that is not a usual manifestation of this disorder. Surgery for TMJ can result in nerve injury.
