Sialolith removal: Sialoliths or  are removed by surgery traditionally. Sometimes you can take medications to increase saliva flow, drink a lot of water to & massage the area manually to push it through.  A newer procedure called sialendoscopy uses tiny  microscopes to find and remove the stones. Find a parotid and salivary gland surgeon in your area who can do this procedure and has done it before.

: The neck issues are almost certainly a co-morbidity and not the cause of your small fiber neuropathy.  About 60-70% of etiologies can be pinned down during the initial testing, but it may take time in other cases.  A second opinion is often useful.

Infection: More information of the history of the swollen salivary glands is needed.   You may have an infection like sialadinitis.   An MRI of the salivary glands will be helpful.
