Not sure: but if its unchanged for 5-10 yrs thats actually a good thing. A US with doppler will tell if its vascular or a cyst. Soft tissue CT scan may also help.

Put ice on it: start with ice. few times a day. do not take NSAIDS-ibuprofen, aspirin, naprosyn. instead, use tylenol (acetaminophen) for pain. Do not wear tight shoes as it will aggravate the toe. U should also wrap it to help the blood get absorbed, but this depends how large it is. Being on Warfarin complicates things too so I also suggest getting INR checked as soon as possible

This : This sounds like a vaginal abscess (infection), they tend to recur. If the area in question does not drain on it's own or you get a high fever see your doctor. The area may need to be incised and drained.

Gum boil, or fistula: Gum boil, or fistula is the drainage establish by the infection to relieve all the exudates.  When we have a chronic peridontal abscess or the endodontal abscess,  the infection must drain by establishing an epithelial tract to the mucosal surface.  This is called the fistula.  Once the drainage establish, the abscess usually does not exhibit any pain.

No STD, see doctor: A "clear bump" that becomes a sore over a few days would suggest herpes. But already having your test results, it must have been a few days with no change. So not herpes, and no other STD would do anything like this. What did your doctor say? If you haven't seen a doctor in person, that's obviously the next step. If any doubt about your doctor's expertise in skin problems see a dermatologist.

Scrotal blood?: You mean that you have clots in your scrotum ?  How old are you?,  for how long you had this problem?, do you have pain, swelling, what is the skin color,  are you on any medication. have you had trauma? one or both sides.
DO you have dilated veins going into the the lower abdomen, are they soft, or hard,
You need a Doppler/ultrasound.
