Spread to hands: This is not a systemic issue, meaning it is not in your blood and does not spread through out your body and lead to serious illness. It may infect the palms of your hands. The infection can be spread to other family members through contact.

Very unlikely: Toe nail and/or foot fungus would not cause to lose a foot unless there were other complicating factors such as severe vascular insufficiency.
Wish you good health! - Have a diet rich in fresh vegetables, fruits, whole grains, milk and milk products, nuts, beans, legumes, lentils and small amounts of lean meats. Avoid saturated fats. Drink enough water daily, so that your urine is mostly colorless. Exercise at least 150 minutes/week and increase the intensity of exercise gradually. Do not use tobacco, alcohol, weed or street drugs in any form. 
Practice safe sex, if you have sex.

Toenail fungus: terbinafine  cream is for topical (skin) antifungal treatment and will not penetrate into the toenail for effective treatment.   Specific toenail products are used due to components that penetrate into the hard keratin structure of the nail.  There are many over the counter products that can be tried, but often  a stronger  doctors formula or prescription strength product is necessary.

The diagnosis : Really can't be made by "just looking". When nails don't look normal many just assume its fungus. Often times it can be subungual keratinazation or bacterial colonization. The only way to differentiate all these is for a doctor to send a specimen to a pathology lab.

Wouldn't assume it: I would never assume you had such a condition for decades. A consultation with a dermatologist may find you are right &amp; treatment designed for that need. It may also reveal you have a hereditary nail disorder or other issue that must be handled differently. A common reason for good treatments not to cure a problem is a wrong diagnosis to begin with.

Lotrimin (clotrimazole): Over the counter lotrimin (clotrimazole) spray works quite well for athletes foot. Nail fungus on the other hand is difficult to treat with topical medications and requires 3 months of oral lamisil(terbinafine). This will need to be prescribed. Liver tests are usually required for this medication.
