blood in stool for three days now....with the most recent bowel, i have become extremely nauseous and light headed. should i visit the emergency room?

Accepted Answer

Yes: Hard to  tell how much bleeding there is and as such the amount lost.  Normally from hemorrhoidal bleeding one doesn't become symptomatic.  the light headed ness suggests  the possibility that there might be a colonic lesion and that bleeding may have occur longer than suspected.  As such you would need a blood count to know if you are volume depleted and require transfusion and colonoscopy.

i have had diarrhea for approximately a week and notice blood in stool for the past 3 days.  i was on 3 different antibiotics back-to-back since nov13?

Stool culture: You need to have stool cultures sent by your doctor. An infection called Clostridium difficile is a very common infection after being on antibiotics. If this is what you have, you need to be treated. Doctor will likely send stool culture and also a special "C difficile toxin" test.

Further evaluation: It is not normal to have blood in the stool. A lot of things can give you blood in the stool such as bleeding from hemorrhoids, diverticular bleeding, infections/inflammations  such as crohn's, ischemic bowel, malignancy etc.  You should see and talk to your doctor about episodes of blood in the stool and you would need to get further evaluations and work-up - and colonsocopy would be one of them.

Probably not serious: A SMALL quantity of bright red blood in the stool of a young person (like you) is usually due to a trivial cause such as anal fissure or hemorrhoid.  It's POSSIBLE that it is due to a more serious problem, but unlikely.  A rectal exam by a doctor can help make the diagnosis.

Go to see doctor: There are many things that can explain painless blood in the stool, from the most benign one - i.e. Hemorrhoidal bleeding to the more serious conditions such as diverticular bleeding,  colorectal cancers etc.. You should go to see your doctor for further evaluation. If you never have a colonoscopy, you should have a colonoscopy now. If that stops, you still need to see your md for further work-up.

Multiple causes: Several things can cause this. At your age, hemorrhoids is probably the most common cause. Internal hemorrhoids in general bleed but don't hurt. Other less likely causes are peptic ulcers, inflammatory bowel diseases and Merkel's diverticulum.

Needs evaluation pls: Blood in stool is a warning sign. Brisk bleeding warrants emergency care. Causes of bleeding may be predictable (e.g. rectal trauma), but evaluation is always appropriate. A rule of thumb (but not absolute): black stool means bleeding comes from upper GI tract; grape jelly stool from small bowel; silver stool from biliary tree; burgundy stool from right colon; red blood from left colon to anus.

Needs Eval: You need to be evaluated. While hemorrhoids can cause blood in the stool or very severe constipation with hard bowel movements, it is very important other things be ruled out. Please be seen by your doctor immediately.
