Culture?: I would follow up with the physician who wrote for the cipro- I wonder if a culture and sensitivities were done on your urine and if it was adequately treated. Sometimes a different antibiotic or a longer course are necessary- be seen and  good luck!

R/O KIDNEY STONE: UTI with Flank pain with those symptoms is concerning for kidney stone or kidney infection. You need a full exam labs urine C&amp;S. 24 urine for stone analysis kidney ultrasound or CT.. And usually empiric antibiotic therapy pending the results. Make sure to strain your urine and try to capture any debris , it may be a kidney stone. Push fluids water/cranberry juice. Also Urology Dr of SX worsen.

It could be due to kidney disease, autoimmune or systemic illness, tb, medication side effect etc. Infrequent spotting could be from hormonal imbalance, ovarian, thyroid or adrenal problems. Endometriosis?: Endometriosis can cause pain in the abdomen and pelvis.  It seems that you likely had a thorough work up if you saw both a urologist and a nephrologist.  Without a physical exam and your complete history it's difficult to diagnose, but pain syndromes should be considered.  Maybe get a 2nd opinion from another gynecologist?
