Many reasons: There are multiple reasons as to why you have pain and/or sensitivity, -- did you have deep decay removed... Was there cracked tooth syndrome diagnosed?... Is your periodontal and gum health in good condition... Is there recession and sensitive root near the margin of the crown... All are important, ask your dentist to check it out and take a radiograph as well, test tooth thermally as well.

Radiograph: A radiograph is an x-ray.  The hurt could be several things.  Two possibilities are root sensitivity from a little root exposure, the other is a gum issue.  Have the dentist check it.  It could be minor.

Generally: Generally by the arch (in other words upper or lower) but most people have the entire mouth done at the same time. Individual teeth can often be whitened, but without examining you, i'm not sure if individual tooth whitening would be your best option. There are other methods to deal with these types of situations (such as porcelain laminates\ceramic crowns). See a local dentist for a consult.

Depends: It really depends on the condition of the tooth, is it otherwise healthy with intact walls?
If so then have the filling replaced so you floss there without a problem .
If the tooth is heavenly restored with large or multiple filling then crowning the tooth would be best.

See below: A compromised tooth (fracture, failing old filling) may need to be covered and protected by a crown.  First, there must be enuf healthy tooth as a foundation.  It may be necessary to do a "core buildup".  Then the tooth is reduced in all dimensions (ground down if you must), a mold (impression) is taken, and then a crown is custom fabricated to recreate the tooth that was once there.

No Sticky Foods: No sticky foods like taffy, jujubes, abba zabba that could stick to the crown and it's opposing tooth and pull the new crown off.  The crown cement needs to set.

Why Is it Drilled: It depends why the tooth is being drilled. If the bite is off, an adjustment of the tooth usually takes off a small fraction of a millimeter of the enamel outer coating. Small areas of decay can be drilled out & a simple filling placed.  When decay is deep, the tooth is fracture or more and 1/2 to 2/3 of the tooth needs restoring then a crown is needed.
