Yes: Even though Depo Provera (medroxyprogesterone) prevents ovulation, your ovaries will still produce follicles that can be seen on u/s.  But these follicles will not release an egg.

No: Your gyn should have explained that to you , you can have follicles on both ovaries and not ovulate at all, and if u do ovulate it is from one side or the other each month , not both.

Normal finding: Ovarian follicles are where eggs develop.  During a normal cycle, many eggs start to develop in these follicles until one of them (or sometimes more, in the case of twins/triplets) pops out.  "popping out" is ovulation.  When many follicles are seen on ultrasound, it just means they were able to see lots of them developing prior to ovulation.

Routine GYN exams: Ovarian cysts tend to come and go as do follicle. These are normal; however, you should check with your gyn again in 2-3 months. Otherwise, these conditions , as you stated, appear to be normal.

Fibroid growth: During pregnancy, large amounts of estrogen are secreted which can cause significant growth of the fibroid, sometimes impinging on the developing pregnancy with a resultant miscarriage. Ovarian follicle growth is a common response to prepare for ovulation when not on a hormonal contraceptive suppressant.

Normal: Sounds like you have been reading an ultrasound report? Bilateral means "both sides, right and left" and follicles are normal ovarian cysts, where eggs live. The word functional is redundant here, but also describes a normal ovarian finding.

Depends: If you are having symptoms from the fibroid then a myomectomy may relieve the symptoms. If you are not having symptoms and especially if you are close to menopause then a myomectomy may not be needed.

Ovulation: In every normal menstrual cycle, there is development of follicles with a dominant one. The recruitment of follicles start about cycle day 2. The ovulation which occurs from the dominant follicle when it is usually more than 18mm to facilitate conception if spermatzoids  are available.
Follicular development is very crucial in getting pregnant.
