Tendon lengthening: If 6 months of physical therapy has not helped, then it is time to discuss surgical intervention.  I assume you have had xrays and or mri?  If not, then you need to be evaluated by a surgical and/or sports medicine trained podiatrist.  You may need to have a lengthening procedure performed on the achilles tendon.

Workup: Usually result of overuse (high impact) activity. It also can be associated with having an underlying autoimmune disease that activity brings it out certain antibiotics have also caused it to occur (cipro/levoquin).See  a rheumatologist to ruleout.
If rest and pt has not helped, bracing, prp (platelet rich plasma) injection can help, shock wave treatment and ultimately surgery as last resort.

Achilles: Typically, when the antibiotic is removed, the tendinosis will improve. There is a spectrum of disease pathology and also treatment options. If it doesn't improve with the removal of the abx, a reasonable option is a walking boot for 6-8 weeks. If bad enough, maybe achilles' tendon debridement with flexor hallucis longus augmentation.

Not unheard of: I've had patients, with this observation following Levaquin (levofloxacin) usage. 15 months of pt was long enough. Consider immobilization via a cam walker and then hit it hard with both an oral steroid and oral nonsteroidal anti-inflammatory. Consider cortizone injection to one side by your local area podiatrist.  Do the other side, if vast improvement putting 3-4 weeks between injections.  Stop living in pain!

Quinolones: Can cause or induce tendon ruptures, I am not sure insertional tendonitis is blamed on quinolones......

Accepted Answer

6-12 months: Depending on the type of surgery and extent of tendon involvement.  Most procedures allow walking within 2-6 weeks of the surgery, with full recovery within the 6-12 month range.  Of course, you should have an informed and extensive discussion with your surgeon prior to surgery.

Achilles tendonitis: You need to be evaluated by a podiatrist or orthopedist.  X-rays should be taken to evaluate the area.  Often times a simple 1/8 to 1/4 inch heel lift will help, but remember if you lift one side you should lift both unless you have a diagnosed limb length discrepancy.  Physical therapy can often times be beneficial with achilles tendonitis.

: Achilles tendinitis can be very difficult to treat. You did not say how are you develop this condition. If it was through running or some other activity because it can make a difference. I would treat runners with Achilles tendinitis using ice massage in the morning and putting ice on it in the evening for about 10 minutes. Anti-inflammatory medication and stretching and strengthening program
