the top of my big toe itches off and on and i scratch until it eventually causes a blister any idea its always the same toe?

Toe itch: This could very well be a fungal infection that is causing the itch which you scratch till the blister forms. I would try a topical antifungal cream and a powder ans see if that helps.

? Callus: If there is thickening of the skin on your toe you may have a callus. Sometimes the area can cause itching. Sometimes a pumice stone may help to remove dead layers of skin and then you can apply some mild hydrocortisone cream. If this does not seem to be the problem, see your doctor for follow up.

Most likely: Most likely a fungus. You can try an over the counter topical like lamasil. If it doesn't seem like its responding after a week I would see a podiatrist.

Fungal infection: Perhaps this is a fungal infection of the toes.  Is the itching worse after wearing socks & shoes for several hours?  Is it worse at the end of the day (assuming you work daytime hours)? Any redness, irritation, peeling skin?  It doesn't have to have a foul odor.  If yes to most of these questions, consider a fungal infection. Try some otc remedies first.  If no improvement, see a podiatrist.

Go to podiatrist: As a diabetic, any new lesion that develops on your foot whether it is painful or not should be evaluated.

Likely: It is probably fungal ( athlete's foot ). Sometimes there is also bacterial infection. Use some OTC creams and keep toe webs and feet dry.

Fungus: I'd be suspicious that you have a superficial fungal infection. It doubt it's related to an ingrown toenail (with removal?) last year. There are many over the counter treatments - look for an antifungal cream like clotrimazole or lamisil (terbinafine).

Insoles: This is probably forefoot overload from the way you walk and the dry skin is actually callous formation from driving your big toe into the ground when walking.  I'd first try an insole. If that doesn't work, see your local foot doctor.  Search www.Aofas.Org if you don't know of one in your area.
