according to your experience what is the best drugs for generalized anxiety disorder and depression and do not increase the prolactin levels, tried many classes but doesn't seems that effects and see many psychiatrists ,so just your advice on drug rx?

: Cognitive behavioral therapy with a trained Counsellor/psychotherapist can often be the most helpful thing, but SNRI meds like venlafaxine &amp; escitalopram, or SSRI meds like sertraline &amp; fluoxetine are usually 1st line therapy. Spiritual Counselling with prayer &amp; Bible reading can also help. Our world is very stressful, especially with COVID added in!

SSRIs: Selective serotonin reuptake inhibitors (SSRIs) are the most commonly prescribed medications for generalized anxiety disorder. These include fluoxetine, paroxetine, sertraline, and citalopram (among others). 
They are more or less equal in terms of likelihood of therapeutic response. 
Ideally, they should be started at low doses since they can sometimes worsen anxiety if the starting dose is high.

It depends: On many factors as to if you'll need to take forever. How you've done in therapy, your family and medical history, etc. How it affects you may depend upon side effects, how side effects are addressed now, dosing, treatment response, etc. 
Please discuss with your provider how you’re doing on your current med regimen. If you're having side effects, he/she needs to know ASAP.

Try psychotherapy: You do not mention what meds you have tried, or what treatments.  If you are using a TENS unit, are you having chronic pain? - this complicates treatment of anxiety.  Have you tried cognitive-behavioral psychotherapy (CBT) from an experienced psychologist?  This can be more effective than medications for anxiety.  Good luck!

A psychiatrist can: answer this based on your family medical history and your medical history: allergies, what meds have worked in the past. They can prescribe a number of meds like Klonopin, Xanax, Prozac, Paxil, (paroxetine) Celexa, Zoloft, and Lexapro. You need to ask a health food store about OTC meds for anxiety. I am unsure of that.

SSRI:  There is no one correct medication choice. I would start with a selective serotonin reuptake inhibitor.

Also, if you have not already done it, you may want to see your Family Medicine physician to rule out any additional medical conditions which may present like anxiety. Talking to your providers about optimizing your treatment (e.g., additional CBT sessions or trying mindfulness-based sress reduction, etc)

Meds + CBT: Medication and cognitive behavioral therapy together are more likely to manage GAD than either alone.   Your fears are not paranoia but a form of anxiety possibly phobia.

CBT Therapy: All research agrees that therapy optimizes the effects of medication. For some it allows you to cut dosage or stop earlier. Do discuss this all w/ your Dr (very wise of you!) but I see little reason to think that a higher dose will impact severe GAD w/o Tx. You need to learn skills to avoid, reduce and manage anxiety.
