Sometime the disease becomes dormant and doesn't require these powerful and sometimes dangerous drugs. A rheumatologist is your best bet to get the right treatment. You want to make sure it is really RA that is being treated, too.

Tmj dysfnctn could be the culprit, and maybe you have problems in the brachial plexus such as thoracic outlet syndrome.  Consider possible nerve or joint pain generators, and see your doctor to get a confirmed diagnosis first, and treatment later.

RA: You are right about the nerve being twisted. Due to the RA your joints are changing and they swell. Increase swelling in your ankles places pressure on the nerves that go through the "expansions" around your ankle. Hence this puts pressure  on your nerves and pinches them causing burning sensation. Atherosclerosis would cause cramping pain  in your calves.
