Varies : There are many potential causes including a phlebitis, which is an inflammatory condition of the veins themselves; can also be an arthritic or inflammatory condition of the wrist or hand itself.  Don’t hesitate to be seen for an accurate diagnosis.

Ivdu valium: I'm sorry to hear that you're struggling so much with what sounds like benzodiazepine addiction. You may have had what is commonly called a "blown" vein. Most importantly, it sounds like you need to enter a detoxification and drug treatment center. IV drug use carries significant risk including hepatitis and HIV. Please think about detox today. Treatment for co morbid anxiety will help.

Hand/arm pain: is it in the finger? which fingers? Do you have numbness? Tingling? Can be carpal tunnel vs thoracic outlet syndrome versus tenodnitis/ or rarely if previsou injury complex regionla pain or clot. Would follow up with doctor/urgent  care

Venous pattern: This venous observation may not be significant unless the veins remain distended when your arms are elevated at shoulder height. If they don't empty at that level, then some sort of obstruction could be present. Have your physician check you if there is any doubt about persistent distended veins in your upper extremities.

loss of fat: Over the years, the subcutaneous fat in our body naturally atrophies.  Think of the difference between the cheeks of a chubby baby and a 90 year old man.  This is also true in the hands.  Some physicians can perform sclerotherapy or ablation of hand veins to make them less noticeable - make sure to go to a vein specialist or phlebologist.

Small Aneurysm?: I saw the video. Most likely is of no concern but if this is a pulsatile artery is very superficial and any trauma can make it bleed fast and easily. Have you or your family had aneurysms? I would check with a vascular surgeon if they thing it warrants to get at least a doppler ultrasound. Other remote possibility is a av malformation.

? phlebitis or lymph: Redness along the arm in a long streak pattern can be related to phlebitis-inflammation of the vein and/or lymphangitis-inflammation of the lymph vessels due to bacterial infection.  This needs physician review.  Can be a big problem with need for antibiotic therapy.
