i have a very painful external hemorrhoid, its a constant pain so i couldnt sleep last night, panados dont help ease the pain,  im using anusol ointment and amethocaine cream since yesterday evening, but its not improving ?

External hemorrhoid : Hemorrhoids sometimes need surgery.  There are several other treatments such as creams, enema, and treating constipation

can i continue taking iron tablets  after a hemorrhoids surgery?  when can i have sex ?

See below: No substantial difference between ointment and cream. You may continue taking iron tablets as prescribed by your doctor. You may have sex when you are ready and feel like having it.
For good health - Have a diet rich in fresh vegetables, fruits, whole grains, milk and milk products, nuts, beans, legumes, lentils and small amounts of lean meats. Avoid saturated fats. Drink enough water daily, so that your urine is mostly colorless. Exercise at least 150 minutes/week and increase the intensity of exercise gradually. Do not use tobacco, alcohol, weed or street drugs in any form. 
Practice safe sex, if you have sex.

: It would be OK if you're also on anti-yeast treatment, like miconazole, fluconazole, etc. HC stands for hydrocortisone, which helps yeast grow as well as reducing inflammation and itchiness.

You need a new drug: As huey lewis says, you need a new drug.  Usually if anusol doesnt work then you need to add hydrocortizone.  The new drug is anusol hc.  It also is available in cream or suppository.

Sits bath/fiber: Sitz baths are helpful for inflammation. Wipe with wet TP. It is also crucial to have soft regular BMS - a diet high in fiber with a fiber supplement may help.  (Dark green veggies like kale, prunes and beets are especially good) There is some evidence Diosmin, which is a citrus supplement may help with venous inflammation, but does have interactions with other herbs and blood thinners.

been using anusol with hydrocortisone for 7 days on piles & it still hasn't shrank or gone back up rectum. safe to keep using cream or try pushing it in? will it ever go back in/shrink? haveadjusted diet, no constipation/straining since finding piles

Short time: Anusol with hydrocortisone is not meant to make them dissapear. Mostly, it will make them less painful or swollen. Diet will make it less likely to have pain or inflammation in the hemorrhoids. However, the only way to make them dissapear is surgery. Not safe to try to push them in, as they will go back out anyway.

Won't work: Anusol cream is not an antifungal and it will not help a vaginal yeast infection. In fact, if it contains steroid cream it may make the infection worse. I would recommend an over the counter vaginal yeast medication.

Hemrrhoids: Generally not on their own.

Anusol and perperatin h havs steroids hence help to calm down inflammation

combination of things with lifestyle change including  high fiber diet ( morethen 25 gm/ day  )  with regular excercise to aid in bowel mobility, adequate consumption of water  and stool softners like docusate sodium help with prevention and to get rid of hemorrhoids.
