year ago i had a strong eczema (due to excessive use of detergents) that affected all my fingers. antibiotic for staphylococcus aureus (yellow blisters) has resolved the problem however from time to time ring finger exhibits itchy, flaking eczema.

Depends: Your definition of severe is difficult to evaluate which will influence oral or IV routes. Furthermore a penicillin allergy may effect the choice as well as community patterns and hospital based organisms. A culture is the gold standard for determining treatment but most cliicians would begin with Cephalexin or Dicloxacillin or amoxicilin/clavulonate and then Clindamycin in pcn allergic patients.

If skin infection of the arm or nearby that is believed to be causing the lymph node inflammation, then staph usually should be covered with antibiotics. The usual approach in that situation is combination treatment, e.g. with cephalexin (Keflex®) plus sulfa-trimethoprim (Bactrim®). Discuss with your doctor.

Some meds like vancomycin & gentamycin are very effective but can require extra blood tests & have side effects. Often the dr chooses the big guns early and drops back to a milder agent if lab tests show it will work in that case.

Overall, most of time vancomycin is used for staph infections of neonates, but again this is a case by case answer dependent on culture results and other treatments and cares your infant is getting.
