Depends on situation: Most hospitals know what kind of germs are around in patients & what meds will best treat them. This helps create a starting point.Location & severity also factor in. Some meds like vancomycin & gentamycin are very effective but can require extra blood tests & have side effects. Often the dr chooses the big guns early and drops back to a milder agent if lab tests show it will work in that case.
