Anxiety : Anxiety is the most common reason people seek psychiatric consultation.  So you are not alone.  Medication can be helpful in to treat the symptoms.  However, underneath the symptoms are the cause of the anxiety, panic, and difficulty sleeping.  Anxiety is usually a secondary emotion to deeper emotions.  Psychotherapy (considered to be therapy anything less than three times a week) and in particular, psychoanalysis (anything three or more times per week) can help understand the deeper emotions and change patterns that will eliminate the anxiety, panic and sleeping difficulties.
  
It also sounds as though you might be coping with add and it may be helpful to be tested and treated for this as well.  Being treated for the symptoms (with medication) can make a tremendous difference in one's productivity and therefore self-esteem. 
The best approach would be to see a psychiatrist that also specializes in psychotherapy.  This way you will get good treatment for your symptoms with medications, but also have the option of psychotherapy. 
 
The psychiatrist with the most advanced training in psychotherapy are those trained in psychoanalysis.  If you choose to look for someone, you may want to ask about contemporary analysis and call a few.  Most of them will allow a phone interview for you to see if you are comfortable with them and if it is a good fit.  Larger cities have psychoanalytic institutes that may offer some names, or reduced fee analysis by trainees that are supervised by senior analysts.

No need to wait: Unless you have significant side effects from your medication, you should be able to exercise shortly after taking them as long as your blood pressure doesn't'  drop too low.

No science on this : Probably not. Osgood schlatter is a condition involving a portion of the tibial growth plate at the insertion of the patellar tendon. It resolves when the growth plates close. The best cure for it is to stretch, stretch and stretch, supplemented with rest, ice and anti inflammatory medication. Save your money and don't buy it; there are no proven benefits and no evidence that is safe.

i have a wound infection on my knee from a grass burn that reopened a few times while playing a sport. the doctor prescribed ranclav 1g twice a day for 5 days. will i be able to exercise lightly while using this medication?

Yes: Light exercise isn't a contraindication while on an antibiotic, however if your doctor asked you to rest the area till done with the treatment, better to not exercise till done and have a follow-up with your doctor, best wishes

Probably not serious: Extrasystolic beats, premature atrial or ventricular contractions are beats that occur before the regular 'sinus' beats; since every heart muscles has the ability to cause a heart contraction, these are not uncommon; 7/day are very few; can occur with mitral valve prolapse, caffeine, smoking...If any question, next should do a stress test to assess if there could be any underlying heart problem.

Depends: Depends on your team and what the protocols are. Some teams may do a drug test with physicals although most will do it randomly.

Generally, No: As part of the recommended sports physical, drug testing is not done.  A urine test may be obtained, but it is testing for blood, sugar, protein, etc.  However, some schools require drug testing for participants in extra-curricular activities (sports, band, etc.).  That is a local issue and, generally, the physician doing the exam is not a participant in the drug screen nor privy to the results.

Eat right.: More protein and fewer carbs! the protein will keep you from losing muscle, while you burn fat.  The typical diet that is hyopcaloric will cause one pound of muscle loss per three pounds of fat loss.  Not good.  With exercise and enough protein, you can add muscle, while losing body fat!

Ignore failure: Motivation to quit is great first step. Discuss with your doctor. Use multiple strategies e.g., nicotine replacement + meds + behavioral approaches. Take one day at a time, and if you don't succeed, ignore failure and try again, and again, and again, until you succeed. Don't let failure sap your self-esteem. If it were really easy to quit, it wouldn't be an addiction. All the best!
