Dental Cleaning: Have you had your teeth cleaned?  The oral bacteria in the mouth are the same bacteria that can cause acne.

It's possible : There are many possibilities.   Try to wash your skin with a mild moisturizing soap 2 to 3 times daily.   There are problems such as PCOS (a complex hormonal problem) that can cause this type of rash.   If careful skin cleansing does not resolve the rash, see a primary care doctor or dermatologist for an evaluation.

3 years is a while: A person with 3 years of rash or other skin lesions on the face can be evaluated by a dermatologist to see what is going on. A primary care doctor can also evaluate, looking at the entire patient to see if there is some disorder that happens to show up as a face rash. Treatments from the dermatologist should be helpful.

It : It sounds like you have something called perioral dermatitis. This can be a common rash in women, especially this time of year. It tends to wax and wane but usually doesn't go away with out proper treatment. Treatment usually consists of a combination of a topical cream and an oral antibiotic. I recommend a trip to your dermatologist to confirm the diagnosis and to get the proper treatment to make you better. Good luck.

Unlikely infection: A staph or secondary infection  Would be more likely to be red, pimple like, and painful.  This sounds more like eczema, and the itchy nose suggests allergic rhinitis. When the skin is dry, the oil gland pores are plugged and cause the white heads.
