Without knowing your exact details and going on the limited data you provided i can only assume you are pregnant until proven otherwise. While irregular periods can be a side effect I have not seen it.

No effect: Isotretinoin does not change your menstrual cycle. That is why it is vital for women of childbearing age to have a pregnancy test before starting to take the medication and a new test every month before starting the next months treatment. You should be using two forms of birth control to prevent pregnancy as this medication can cause severe abnormalities in a developing fetus.

Within 3 months: Acne can flare the first few months on isotretinoin.  Most patients start to improve by 12 weeks.  There are always a few patients who require more time to improve.
