Shock-like pain : You need a go neurological examination. The most common reason for this is something compressing the spinal cord, it can be from damage or lesion within the spinal cord. See a neurologist

Yes: The genitofemoral nerve is responsible for both the efferent and afferent limbs of the cremasteric reflex. The ilioinguinal nerve (l1) is also involved in the afferent limb of the reflex. So it is possible for some for the reflex to return in time with some nerve regrowth and sprouting, but most will lose this reflex.

: Hard to sensibly tie all this together.  The  leg numbness, if inner leg, would fit a saphenous neuropathy, a benign sensory problem unrelated to the reflexes.  The triceps absence may relate to a  C7 neck lesion.  The brisk  leg reflexes could be due to neck problem or heightened anxiety.  Need much more information to make sense of this.   See a neurologist and get fully assessed.

in inguinal hernia repair surgery if some of the cremasteric fibres/muscles gets excised will it cause reduced/absent cremasteric reflexes that side?

Possibly: If all the fibers were to get cut or removed you would not have a cremasteric reflex.  Sometimes the muscle may heal back together and i would guess the reflex may return or strengthen.

: First, the best way to interpret this is to inquire directly from the physician involved.  Cachexia is chronic wasting and loss of bodily bulk.  Mentioned are focal muscular problems in the right hand and left hip.  The reflexes suggest peripheral neuropathy.  But all of this needs clinical correlation.  If this is you, or a relative, don't you have a right to further understanding?
