ive scratched the skin on my lower back near my buttocks to the point where the open wound is white and bright pink and doesnt bleed but its extremely painful. should i be concerned and if so what should i be concerned about this layer of skin.

Call you doctor: If you are scratching your skin to the point of breaking down your skin, you need to have an exam. It could be simple but you must rule out more serious diagnosis. Call your doctor, they can even help with telemedicine if you can shoot a picture of the area.

I advise you: to leave it alone and see a doctor for proper diagnosis.  Meantime, just use a bland moisturizer like vaseline a few times a day.

Herpes simplex: Can be early symptoms of herpes outbreak. When this subliminal pain occurs it is not always followed by full blown lesions.  It is not clear why different presentations occur in different individuals.

Sure: After any injury to the skin there will be discoloration. This can take many months to go away.

Cyst with abscess: Sounds like a pilonidal cyst and this can develop infection as bacteria track into the area from a tiny opening into the gi tract.  This needs  to be seen and treated by your doctor asap to prevent worsening and more pain for you.  See your doctor soon and hope you feel better soon.

Intergluteal rash: Two of most common rashes between buttocks are psoriasis & eczema/lichen simplex chronicus (lsc). Lsc results when itchy skin is rubbed/scratched & gets thicker, more scaly & itchier as a result.  Topical treatments can help but all friction in the area must stop. Blot, not rub with towel. Use wet toilet tissue to wipe, dry to blot, use fragrance free cleansing bar like cetaphil, no wash cloths.

Possible fungal.: Usually because that area is hidden and moist many fungus harbor there and can cause an infection. This is very common especially during the summer months due to the heat. you may try over the counter clotrimazole( an anti fungal cream) for example and see how you respond in a few days. If rash persists or gets worse you will need to see your doctor for a formal evaluation and treatment.
