Maybe: We really need more information. It would be unusual for a four year old to be normally losing a tooth. More likely some pathology is involved and then yes the tooth should be extracted. That said sometimes we allow loose teeth that are going to be lost prematurely to fall out on their own. This situation most happens when a tooth has previously been restored with a pulpotomy and crown.

Yes: Juvenile teeth can start shedding anywhere from age 4-6 and end anywhere from age 10-12. The important indicator is that there is an adult tooth underneath ready to erupt. Talk to your dentist if the tooth falls out and there is no sign of an adult tooth erupting.

An oral surgeon will be able to comfortably and safely sedate your child for this procedure - so that it will not turn into a "bad experience" and lasting negative impression for them.

: It most likely would drop out and replaced by the adult teeth. It is however important to make sure that you give nutritionally balanced meal and importantly vitamins.

No: Probably viral, and totally unrelated to filling, unless tooth is infected. High fever and shaking ire disturbing. See both Dentist and Physician to try to determine cause of infection and appropriate treatment. Keep her hydrated and try to bring down the fever.

Types of fillings: The best type of filling for a 4 year old would be a properly placed and restored one by a competent dentist, preferably a Pedodontist. Amalgam fillings and composite fillings are both acceptable. Your own dentist can determine what he\she feels would be the best one to use depending upon the individual circumstances.

RCT or Extraction: Whether to do RCT or extract a tooth depends upon many factors and not purely the age of the patient (which is the only information given to us). It depends on whether it is an upper or lower tooth, an anterior or posterior tooth, infected or not, integrity of the tooth, occlusion, the development of the permanent tooth etc. A Pedodontist and Orthodontist can confer with each other and advise you.

my 4 year old daughter has a cavity in the upper canine, the dentist has advised to to an rct for the tooth, since she believes that the nerve has been damaged. she also said we could do a tooth removal, but that would leave a space, what should i do?

: Follow your dentist's advice and have root canal therapy performed. That tooth usually exfoliates around 11 years old and is needed to maintain  the spacing of the adjacent teeth to allow for proper secondary tooth eruption. Premature loss can lead to the need for future orthodontic care. Moving forward have her teeth checked and cleaned every six months.

if i have both oral and genital herpes and my 4 year old son used my tooth brush by accident while i was using it he grab it does he have it now?

It depends...: If you have the active phase of the disease, break out in your mouth or lip, then it is possible. But if he has strong immune response, he may be protected. It is better to keep your tooth brush away from your family especially in the active phase of the disease, change tooth brush immediately after. Also ask your physician or dentist if Valtrex (valacyclovir) can help you.
