Probably viral: A COVID 19 screening test may be indicated. Please use good hand washing and disinfect surfaces.The other possible cause is a viral gastroenteritis infection. Continue to breast feed, keep the baby well hydrated, check his temperature, and please follow up with your PCP. The baby's usual immunizations should be provided. If there is a fever of 101 or higher, please see the urgent care center.

Viral Syndrome: The two most common organisms that infect humans are bacteria and viruses. Bacterial infection generally gets worse as the hours and days pass, however, viral illnesses especially the common cold will start with vever that resolves in 2-3 days and then congestion and cough and finally resolution. This was probably a viral upper respiratory infection or other viral illness.

No cough medicine: All cough syrups are contraindicated in babies. If the cough is not severe, you may try to rub Vicks on the soles of the feet (Canadian study) and then put on socks, works very well for about 4 hrs.
If the cough worsens, see his pediatrician in case it might be due to RSV or flu virus, or bronchitis.

Mgt/workup: I would advise using gentle infant nasal saline. If her cough is moderate to severe, causing gagging or interfering with feeding, she should be evaluated. The testing can include Pertussis, RSV and community acquired viral panels, in addition to considering bacterial causes. 
Monitor her temperature, and please let your physician know if she has a fever.

Watch for now: 2-3 Normal Saline drops to the each nostril few times a day with a bulb suction afterwards; more frequent breastfeeding: q 2 hrs,  to avoid dehydration and provide more protection from the maternal breast milk. If you noticed temperature elevation &gt;100.4; behavioral changes: sleep pattern, feeding, irritability, cry - which are getting worse; decreased amount of wet/soiled diaper changes -see peds.

No meds: 3 month old infants are too young for any kind of medication decongestant. The best way to unblock her nose is mechanically. While she is laying down, let 2-3 drops of nasal saline solution drip into her nostril. Wait 5-10 seconds, then aspirate out loosened mucus with a bulb aspirator.  Repeat this for each nostril every 2-3 hours and just before feeding.

No! And how on: earth can you tell if a three month old has reflux? It is normal for an infant that age to regurgitate and spit up feedings. You need to take the infant to the pediatrician and discuss your concerns before you do something to it which is potentially harmful!
