Many possibilities: Hydrocele is common, is a sac of fluid around testicle. Left testicle can be smaller if varicocele present. Testicle can be smaller if it was undescended in spite of corrective surgery. One testicle can be minute if it underwent torsion prenatally or in infancy. Possible sequela of mumps. Tumor or cyst can make that testis larger. Finally some men have one tetis larger for unknown & benign reason.
