Coital bleeding: Bleeding during intercourse is abnormal but causes can be as benign as dryness or can relate to std like gonorrhea or chlamydia. Benign growths like polyps can bleed and of course, cancer of the cervix or vagina can bleed with intercourse. Only a thorough vaginal exam can discern the cause of bleeding.

Yes: Vaginal tissue is very compliant or flexible. One method is through lubrication which usually takes a little time. Without the lubrication, which does happen naturally, there can be flesh tearing which is never good.

Hmmm: Doesn't sound like it. Sex should be pleasurable and tears doesn't sound happy. This is a really hard topic. You should talk to your partner, your doctor and maybe a counselor. It is important to figure out why: is it painful? Are you scared? You should probably work it out before you try again. Waiting doesn't hurt.
