Phimosis: One can use a cream called Elidel (pimecrolimus) which is a prescription, but best course of action is a circumcision, especially at your young age.

Yes: Yes that is a problem and should be fixed. Depending on how small the opening is sometimes those things can be stretched but surgery is the better solution to fix that problem. It should be a minor operation to fix the foreskin and relatively little discomfort.

Yes: The problem of symptomatic phimosis can often be cured by applying a strong steroid cream (eg betamethasone) to the retracted foreskin twice daily for up to 4 weeks. Once the prepuce is retractile, it should be retracted with each urination and with bathing to diminish the chances of recurrence. More invasive alternatives include stretching/dilating under ansthesia or incising the phimotic ring.
