Anal fistula by definition is infected due to contact with fecal matter. Diet will not prevent infection. You need to see a doctor for proper treatment.

Anal fistula is infected however if the skin closes over on the external opening, you can develop an abscess that will require surgical incision and drainage. Keep the area clean and not moist from perspiration but best to see a colorectal surgeon for evaluation and consideration of any underlying disease like inflammatory bowel disease which will need to be addressed promptly.
