If genital herpes, which seems likely, he needs immediate treatment with anti-herpes drug (valacyclovir or acyclovir), and professional evaluation in person for proper diagnosis. Oral herpes infection without cold sore outbreaks is very common. It will never be possible to be sure of the problem or to know the source of his infection on this or any other online forum.
