I am considering adult male circumcision but have hsv-ii. i take valtrex 500mg/day and no outbreaks in 5 years. should i be concerned that the surgery could create a severe outbreak even with valtrex and avoid the procedure altogether?

If indeed you have HSV2, this is a reasonable request, since local trauma can trigger HSV outbreaks. Your current dose of valacyclovir should be very effective, but might increase protection (and would not be harmful in any way) to take a larger dose of valacyclovir, such as 1 gram twice daily for 3-4 days starting the day before circumcision.
