Never ever will i have my ureteral stent removed or a vasectomy performed while physically, mentally or psychologically conscious of it . how can it be deemed medically necessary for insurance to cover general anesthesia? it is a must no question?

Certainly work with your insurance company and request a review. Also, check your contract. Have your doctor challenge the decision, if adverse. All states have an insurance commissioner, and you could petition the office for help, if no progress.
