I have had severe anal and vaginal itching for a few months, only external itching and it is severely bad at night especially. there is also an on and off foul smelling odour and discharge from my vagina but comes and goes with my cycle.

Enterobius vermicul: This could be Enterobius vermicularis infestation (which could affect all family member) vs yeas infection. I advise for personal hygiene and have physical exam . you may try Diflucan 150 mg po and if improved take another 150 mg in 72 hours

Vaginitis: May be a yeast infection. Would follow up with MD for Diflucan ( as since it is long standing vaginal creams like Monostat may not work) and testing for other vaginal infections like trichomonas, chlamydia, BV, ureoplasmasma/ mycoplasma to be complete. ,
