Not likely: A fistula is not likely in this situation. When you pass gas, it has to go somewhere. If you're sitting down, it's likely some of the air travels *ahem* forward and causes that sensation. If you're worried, your GYN take a look. Fístulas aren't very common in young, healthy patients.
