Here are some...: Are you sure that scrotal itchy spots are related with skin fungal infection? If not improving under fluconazole as expected, try applying topical cream with anifungal agent with 1% hydrocortison gingerly 1-2 times daily and keep the affected area as clean & dry as possible without using much soap or cleansing soap since contact dermatitis, atopic dermatitis, psoriasis, & dermatomycosis are common
